Your First Visit
Your first visit is a great time for us to sit down and get to know each other. You’ll be warmly welcomed by our friendly office staff, and they’ll help you get your paperwork processed. You can find your New Patient Forms located below so you can fill them out before you come in to save time and stress in the office. We’ll give you a tour of our office and you’ll sit down one-on-one with Dr. Patel.
We want to get to know you on a personal level so we can provide the very best care for you, so don’t be shy, we want to know everything. This is a great time for you to ask any questions you may have or let us know what your fears, concerns, and goals are. Insurance & Financing
Hamburg Family Dental knows it can be difficult to incorporate funding dental care with everyday expenses, so we work hard to remove any barrier that could prevent you from seeking necessary dental care. We’re happy to accept most major dental insurance plans and offer flexible financing options to assist you with prioritizing your oral health without breaking the bank.
Dental Insurance
Insurance companies, also sometimes called carriers or providers, typically reimburse a portion of our fee. Their portion is determined by a contract between the person or entity that pays your premiums, whether that is you, your employer, your spouse’s employer, or someone else. Hamburg Family Dental processes your insurance claims as a courtesy to you. We will always work with your carrier to ensure you receive maximum insurance coverage and keep your out-of-pocket costs as affordable as possible. You can always feel confident that we will provide our patients with exceptional service regardless of limitations imposed by insurance coverage.

We accept payments made with flexible spending accounts, health savings accounts, cash, check, Visa, Mastercard®,
PayPal, Google Pay, and Venmo.
The HFD Copper Advantage Plan
If you don’t have dental insurance and are looking for alternate ways to defray costs of dental care, we are proud to offer our own HFD Copper Advantage Plan. For one reasonable annual fee, you can save on routine cleanings, examinations, x-rays, fluoride treatments, whitening and other procedures. There are no plan maximums, no waiting periods and no exclusions.

$260/year Additional Periodontal Package (For Qualifying Perio Patients)
Annual Membership Includes
- Two Complete Exams (New Patient, Periodic, or Emergency) by Dr. Patel
- One complete annual required radiograph series (4 Bitewing X-rays & 2 Periapicals)
- Two Routine Cleanings* (every 6 months, not including periodontal treatment or deep cleanings)
- Two Fluoride Treatments
- Two Oral Cancer Screenings every year
- 15% off Extractions, Crowns, Partials, Dentures and Mouthguards
- 20% off All Other Procedures: Sealants, Fillings, Periodontics, Additional Cleanings
- $99 Custom Whitening Trays Plus One FREE Whitening Gel
Financial Policy
At Hamburg Family Dental, we want to help ensure you and your family get the quality dental care you need. We strive to be in-network with most area insurance providers and offer flexible financing options that best fit your budget. We encourage new patients to call in advance to firm up insurance details and receive a courtesy benefits analysis from one of our team members if possible. We will attempt to provide you with a treatment estimate prior to scheduling your procedure.
Payment is due when services are scheduled unless prior arrangements have been made. For smaller procedures, full payment will be collected on the date of service. If a service requires multiple office visits and you are unable to pay when scheduling, we will work with you to calculate and collect a predetermined down payment on the initial appointment service date. Patients are responsible for all applicable fees.
When patient accounts become overdue, Hamburg Family Dental sends you a financial statement. Most insurance companies respond within 4-6 weeks. Please call us if the statement we send you does not reflect payments made by you or your insurance provider. Any remaining balance once your insurance remits their portion is the financial responsibility of the patient. When necessary, short-term monthly payment plan arrangements can be made, but we encourage you call our office to see if you qualify.
